Position Summary
Sovah Health is looking for the ideal candidate who will assume principle responsibility for behavioral health level of care assessments for individuals presenting to the Emergency Department in psychiatric or emotional crisis situations and for other patients referred for psychiatric evaluation. Assumes principle responsibility for facilitating placement of patients in need of inpatient psychiatric care. Requires flexibility in scheduling to insure 24/7 on-call coverage for Sovah Health.
• Collaborates with members of the health care team/patient/family and communicates information specific for formulation of an age-specific interdisciplinary plan of care.
• Performs interventions accurately, concisely and timely in accordance with licensure/certification status, standards of care, age and status of patient.
• Documents subjective and objective findings of patient progress. Monitors and modifies the plan of care as needed. Seeks consultation as necessary.
• Educates the patient and family/caregiver about patient deficits.
• Maintains knowledge of treatment modalities that promote stability and mental health recovery.
